It is indeed worthwhile to be aware of all needed documents, applications, fees, and steps and prepare yourself properly in advance, as in many cases, a trip to a Polish Consulate is quite an effort. Here is a step-by-step guide on **Polish passport application**: 1\. REQUIRED DOCUMENTS - Polish citizenship confirmation (DECYZJA) - Polish birth certificate - Polish marriage certificate - If relevant – official name change certificate - PESEL application - Polish passport applications - Passport photos The Polish passport application does not exist in PDF or any other electronic form and cannot be downloaded online. **They only exist in a special form (printed on a specific size carton paper) and are available only at the Polish Consulate or in passport offices in Poland**. We will of course prepare all the above documents, forms, applications (excluding the photos) in advance while processing the citizenship confirmation. With our fully prepared passport file, you will have absolutely no problem filing the applications. There is no room for even the smallest error in neither the documents nor the applications. These are primary civil acts and based on them the passport is issued. 2\. VISIT REGISTRATION Everyone who wants to apply for the Polish passport must **first book a visit** to the Polish Consulate at their **\[**[**official website**](http://www.e-konsulat.gov.pl "Consulate's website")**\]**. Without booking the visit first, it is not possible to apply. In some very busy Polish Consulates in the world like in Tel Aviv, Curitiba, or Buenos Aires, the registration should be done in advance. Everyone must apply in person. It is not possible to send the application and documents by post or to be represented by someone else, as today Polish passports are biometric and fingerprints of the applicant must be taken at the Consulate. 3\. PESEL **Due to the latest changes in the Polish PESEL and civil acts system that is being introduced for the last months there is a new demand by Polish Consulates to apply for the PESEL No. first and only after it is granted for the passport.** 4 . NAME CHANGE People that after emigration officially changed their name(s) in a foreign country must go through the **official name change procedure again in Poland**. In this case, the PESEL number is needed first. The procedure for a name change is quite a complex one and takes a minimum of 3 months. It is a matter of time and formality, though, as the process usually presents no problems. 5\. TEMPORARY PASSPORT In some cases people are in a hurry to get the passport. The solution is to apply for a temporary one. Consuls usually do not make problems with it and issue temporary passports even on the same day. Since Poland entered the UE and with the substantial improvement in the system of Polish passports, the regular 10-years passport is issued quite fast, in 1 – 2 months or so in most cases, so taking a temporary one is not often necessary. 6\. PASSPORT PHOTOS Passport photos must be taken at the most 6 months before applying for the passport. These and Your signature is actually the only thing You need to do by Yourself. All the rest of the documents and forms are prepared by us. 7\. FEES Here you will find all information about **\[[OFFICIAL CONSULAR FEES](http://newyork.mfa.gov.pl/en/consular_information/fees/ "CONSULAR FEES")\]**. It is worth studying these fees and prices in details, especially the costs of translations, certifications, and the Polish citizenship application. — [CHECK YOUR ELIGIBILITY NOW](https://website.polishcitizenship.pl/elegibility/) --- ### [CONTACT](https://polishcitizenship.pl/contact/) **Published:** July 30, 2014 **Author:** Roman **Content:** Please briefly describe your family’s emigration history in the contact form below. **Every case will be carefully analyzed and if necessary consulted** with the relevant Polish authorities with whom we have been closely cooperating for many years ([General Governor of Masovia](http://mazowieckie.pl/en/for-foreigners-1/citizenship/confirmation-of-posses), Ministry of the Interior, Civil Registry, State Archives). Foreign documents proving that the **chain of the citizenship** was not broken in subsequent generations, and 3. Documents that prove/establish the **family line**. 1\. The crucial issue in each administrative procedure for the confirmation of citizenship is relevant **POLISH DOCUMENTS** through which it is possible to prove your ancestors were Polish citizens. According to the applicable Polish law, an application not supported by any of the relevant Polish documents will not be even approved for processing. Therefore, in the event that a person applying for the confirmation of citizenship does not have Polish documents, it is necessary to start to search for them in archives in Poland, Germany, Belarus, Ukraine, or Lithuania. The most important documents are as follows: - Proof of residence, residence registration cards - #### **Extracts from Registers of Permanent Residents (also from before 1918)** - #### **Electoral rolls to the Polish Parliament and local authorities** - Military enlistments - Entries in real estate registers - Notarial deeds - #### **Polish civil acts: birth, marriage, and death certificates** - Documents that prove the confirmation / recognition of Polish citizenship - Certificates of good standing / police clearance certificates - Documents that prove the course of military service and employment - Medical, school and other official certificates - #### ****Copies of Polish ID cards, passports, travel documents, military books, and other identity cards**** It is not easy to conduct searches for archival documents, particularly in territories that before WWII were Polish territories and now are part of Belarus or Ukraine. It has taken us many years to develop appropriate methods of searching for documents by establishing cooperation with various archives and institutions in Poland and abroad. Consequently, we are able to localize and obtain certified copies of relevant documents within 3 months from commencing archival searches. — 2\. It is necessary to prove that there was no loss of Polish citizenship as a result of emigration and naturalization in another country. Relevant documents produced by the country in which the relevant ancestor lived and finally settled are: - naturalization acts (also of previous generations) - documents proving whether ancestors acquired foreign citizenship - **documents of military records until January 19, 1951** - documents proving jobs held until that date — 3\. Usually, the most easily obtainable documents are those proving the family line from your Polish ancestors to you. These are: - Birth and marriage/divorce certificates - Parents’ birth and marriage/divorce certificates - Name change certificates - Certified copy of your valid foreign passport It must again be clearly stressed that the most important part of the process is indeed **Polish documents** that prove ancestral Polish citizenship. Without these documents, it is not possible to apply for citizenship due to new 2011 legislation mandating that any application that is not based on Polish documents will not be accepted for processing. Documents proving that citizenship was not lost are, of course, also crucial.
